1. RENEWAL OF PASSPORT 

Requirements

  1. Application Form
  2. Personal history form(CV) (One photo)
    (It is also required to take the photo at Embassy to be used in MRP Passport)
  3. Copies and Originals of income tax payment receipts
  4. Original and a copy of Passport (Photo page and validity page)
  5. WP/Spass/EP/PR Card (Original and Photo copy)
  6. List of Households/Birth Certificate/Marriage Certificate(if applicant is a dependent)
  7. Copies and Originals of Passport, IC and Tax receipt of your host (if applicant is a dependent)
  8. Recommendation Letter issued by University/ College/ School concerned and
    Student Pass(Copies and Originals) (if applicant is a student)

Note : Myanmar Passports holders and their dependents are eligible to apply for a new Passport when there are only a few pages left.

2. LOSS/DAMAGE OF PASSPORT 

Requirements

  1. Application Form
  2. Personal history form(CV) (One photo)
  3. Copies and Originals of your receipt of income tax (if applicable)
  4. Copies of first page and last endorsement page of your lost passport
  5. WP/ Spass/ EP/ PR Card (copy and Original) (if applicable)
  6. List of Households/ Birth Certificate/ Marriage Certificate(if applicant is a dependent)
  7. Copies and Originals of Passport, IC and Tax receipt of your host (If applicant is a dependent)
  8. Recommendation Letter issued by University/ College/ School concerned and
    Student Pass(Copies and Originals) (if applicant is a student)

3. NEW BORN BABY PASSPORT 

Requirements

  1. Application Form
  2. Birth Certificate
  3. Personal History of new born
  4. Personal History of parents
  5. Marriage Certificate of parents
  6. Parents' passport(Copies and originals)
  7. Tax receipts of parents(Copies and originals)

4. SEAMAN TO SHOREJOB  

Requirements

  1. Application Form
  2. Jop Assigment Letter (Copy and Original)
  3. CDC (Copy and Original)
  4. Original and a copy of Passport (Photo page and validity page)
  5. Personal history form(CV) (One photo)
  6. Tax receipts/Allowment(Copies and originals)

5. INCOME TAX 

Requirements

  1. Application Form
  2. WP/Spass/EP/PR Card (Copy and original)
  3. Original and a copy of Passport (Photo page and validity page)
  4. Tax receipts(Copies and originals)
  5. Tax receipt of Inland Revenue Authority of Singapore
    (if you paid income tax to Singapore government)

6. RECOMMENDATION LETTER FOR BEING STUDENT 

Requirements

  1. Application Form
  2. Student Pass (Copy and original)
    (If applicant has gotten any degree/diploma, certificate is required to submit)
  3. Recommendation Letter issued by University/ College/ School concerned and
    Student Pass(Copies and Originals)
  4. Original and a copy of Passport (Photo page and validity page)

7. RECOMMENDATION LETTER FOR BEING DEPENDENT 

Requirements

  1. Application Form
  2. Passport
  3. Original and a copy of Passport (Photo page and validity page)
  4. List of Households/Birth Certificate/Marriage Certificate

8. RECOMMENDATION LETTER FOR BEING UNEMPLOYED 

Requirements

  1. Application Form

  3. Singapore Immigration White Card(Departure Card)

9. RENOUNCEMENT OF CITIZENSHIP 

Requirements

  1. Application Form
  2. Personal history form(CV) (One photo)
  3. Immigration Letter Copy
  4. Original and a copy of Passport (Photo page and validity page)
  5. Tax receipts(Copies and originals)
    (If applicant is a dependent, you must present tax receipts of your host)

10. AIRTICKET FEE DEPOSIT  

Requirements

  1. Application Form
  2. Original and a copy of Passport (Photo page and validity page)
  3. Original and copy of IC card)
  4. Income tax receipts(If applicable)
  5. Passport and IC of Spouse

11. INVITATION OF MONKS 

Requirements

  1. Application Form
  2. Copy of Passport of applicant
  3. Copy of Passport of venerable
  4. Copy if IC Card(WP/SPass/EP/PR) of applicant
  5. Photo of Buddhist Religional ID Card

12. REGISTRATION
